I think the last to do it was Middlesborough in 2006, in the Uefa Cup Final, I think it was against Sevilla.

The others I can think off are, Norwich,Leeds,Southampton,Notts Forest,Derby,Millwall,Sheff Wed,Leicester,WBA and QPR

Ok then Middlesbrough is correct as is the year.

Of the others they are all correct but no years are given.
